Your health care decisions, in trusted hands.
Choose the person(s) you trust to make health and personal care decisions if you’re unable to, and set out your own wishes in advance, so your family isn’t left guessing.
What a Power of Attorney does
A Power of Attorney is an important part of your Personal Planning, and arguably the most important document you can have. It lets you choose in advance who can manage your legal and financial matters if you’re unable to, or simply not available. We walk you through each decision in plain language.
Key decisions:
-
You choose someone you trust, your Representative, and we also help you name an alternate in case your first choice can’t act.
-
Your Representative can make health care and personal care decisions for you if you’re unable to make them yourself, such as medical treatment and living arrangements.
-
An Advance Directive lets you set out your own wishes for specific treatments you would or wouldn’t want. Not everyone needs one, and we’re happy to talk about whether it’s right for you.
If you don’t have these documents
If you don’t have a Representation Agreement, health care providers turn to a legal hierarchy of decision-makers, typically your spouse, then close family. That may not match who you’d choose, or may leave no clear decision-maker at all.
